How to Enroll:

  • Call (313) 291-2713 for more information, to schedule a visit, or to make an appointment. 
  • We require all participants to complete an intake and assessment interview with the caregiver and the patient. 
  • Our case manager will work with the family to determine a schedule to create a successful experience for your loved one.
